The Challenge

The client operated a network of care management programs running on four separate systems — an EHR for clinical records, a care coordination platform for case managers, a billing and claims system, and a patient-facing engagement app — none of which communicated with each other. Staff were re-entering the same patient data up to four times across systems, errors were introduced at every handoff, and critical care gaps went undetected because no single system had a complete view of the patient. Nightly batch exports created data that was always hours stale, and the manual reconciliation process consumed two full-time staff members. The business needed the systems to behave as one coherent platform without replacing any of them.

Our Approach

We designed a central integration hub — a HIPAA-compliant middleware layer — that connected to each system via its available interface: HL7 FHIR APIs for the EHR, REST APIs for the care and engagement platforms, and a secured SFTP pipeline with transformation logic for the legacy billing system. Every data event (new patient, updated diagnosis, care plan change, appointment, claim status) was normalized into a canonical data model and routed to the appropriate downstream systems in real time. We built a monitoring dashboard for the operations team showing sync status, error queues, and data lineage — so staff could see exactly where a record was and whether it had propagated correctly. Conflict resolution rules were defined and codified for cases where two systems held different versions of the same field. All data in transit and at rest was encrypted, and a full audit log was maintained for every sync event.
